چکیده
PurposeTransplant center volume is associated with improved survival after isolated heart transplant, but its impact on multiorgan transplant outcomes unknown. This study examines the of institutional outcomes.MethodsAdult patients undergoing first time from 2011-2021 were identified in United Network for Organ Sharing database. For each year, centers classified as low-, medium-, or high-volume if they performed <3, 3-5, ≥6 transplants that respectively. Volume cutoffs defined by tertiles 2021. Graft failure was death re-transplantation.ResultsDuring period, 1817 at 104 centers, including 477 (26%) 597 (33%) and 743 (41%) centers. The proportion increased 10% 2011 to 62% Recipient sex, race, body mass index did not vary (all p>0.05). Patients more likely be intensive care unit pre-transplant (59% vs. 44%, p<0.001) have shorter waitlist times (46 94 days, than those low-volume higher combined medium- compared both 30 days (96% 93%, p=0.004) 1 year (88% 84%, p=0.020) post-transplant. Compared low volume, medium high protective against 30-day graft univariate (hazard ratio 0.72 [0.55-0.95]) multivariate 0.53 [0.33-0.84]) Cox regression. Total predictive (p>0.05).ConclusionHigher total early transplant. Transplant outcomes. Adult re-transplantation. During (p>0.05). Higher

Volume-outcome relationship in pancreatic surgery.
BACKGROUND Volume-outcome relationships related to major surgery may be of limited value if observation ends at the point of discharge without taking transfers and later events into consideration. METHODS The volume-outcome relationship in patients who underwent pancreatic surgery between 2008 and 2010 was assessed using claims data for all inpatient episodes from Germany's largest provider o...
متن کاملReevaluation of the volume-outcome relationship for pediatric cardiac surgery.
Over the last 3 decades, a growing body of work has demonstrated a relationship between volume and outcome for a variety of complex surgical procedures and medical conditions. Two large-scale reviews have systemically evaluated the methodology and results of many of these studies across a broad range of conditions.1,2 In 2000, Dudley et al1 found that, among 128 studies examining 40 different p...

From March 1988 to March 1990, 11 children with cystic fibrosis (age 5-15 years) underwent combined heart-lung transplantation at our institutes. Maintenance immunosuppression consisted of cyclosporin and azathioprine with corticosteroids and antithymocyte globulin used perioperatively and during rejection episodes. Six patients (55%) survive from 1.5-23 months all of whom have improved life qu...

Background and Objective: Cystinosis is a rare inherited disease that leads to renal failure. Fanconi syndrome is the major renal involvement in cystinosis patients. Renal transplantation is the treatment of choice in cystinosis children with end-stage renal disease (ESRD). The study aimed to assess the outcome of renal transplantation in Iranian children with cystinosis. Methods: This...
متن کاملThe volume-outcome relationship in hospitals - Lessons from the literature
Concentrating the supply of hospital activity in larger hospital units is often presented as a means to improve the quality of care, but the extent of the relationship and the direction of causality between hospital volume and health outcomes is still a matter for debate. The literature review carried out by the IRDES shows that for certain procedures and interventions, particularly complex sur...
